Bilingual Care Coordinator, LPN

Responsibilities
- Perform monthly clinical check-ins and CCM touchpoints for assigned patient panels
- Conduct structured symptom assessments and evaluate changes in condition using clinical judgment
- Identify clinical red flags and escalate urgent issues via defined triage pathways
- Collaborate with provider teams on symptom changes, medication issues, and care coordination
- Reinforce provider-developed CKD and chronic disease care plans
- Document all patient interactions accurately in EHR and CCM platforms
- Track and log qualifying CCM time for billing compliance
Requirements
- Active LPN or Licensed Vocational Nurse (LVN) license
- Bilingual in Spanish and English
- 1–2 years of clinical experience in ambulatory care, population health, care coordination, or case management
- Proficiency with MS Office Suite (Outlook, Teams)
- Ability to work in a remote environment with a dedicated, private workspace
- Reliable high-speed internet (min 25 Mbps download / 10 Mbps upload)
Preferred Qualifications
- Experience in chronic disease management
- Prior experience in virtual care, remote nursing, or telehealth programs
- Familiarity with CCM regulatory requirements
- Strong patient education and motivational interviewing skills
