Nurse Care Manager

Responsibilities

  • Manage overall care management for an assigned patient panel
  • Establish trusting and empathetic relationships with patients and families
  • Serve as an advocate and community liaison for patients navigating the healthcare system
  • Perform assessments to identify social determinants of health and individual needs
  • Coordinate interdisciplinary approaches to achieve continuity of care
  • Monitor and evaluate the effectiveness of care management plans
  • Collaborate with physicians, APPs, and other clinical disciplines to manage integrated care plans
  • Facilitate patient and caregiver education on treatment options
  • Support seamless transitions of care between different settings
  • Participate in clinical huddles and patient care conferences
  • Engage in organizational process improvement and data collection

Requirements

  • Associate degree in nursing
  • Current RN License (Compact License preferred)
  • Care management experience
  • Intermediate skills with MS Office Suite (Outlook, Teams)
  • Ability to work in a remote environment with high-speed internet
  • Dedicated workspace to protect PHI/HIPAA information

  • Certified Case Manager
